• Bank account details

• Superannuation Choice details

• Curriculum Vitae

• Certificates of all courses undertaken

• A record of your hours worked as a nurse, a Blue Book or a Statutory Declaration, Letters of Service from previous employers

• Immunization History

• Proof of Mantoux

• Proof of Hep B Titre level

• Knowledge of their infectious disease history
