All staff need a 100-Point Identification Check
The 100-point identification check must be completed prior to lodgment of a NSW Criminal Record Check or Working with Children Background Check. During your interview we are required to sight your original identifying documents. The point score of documents produced must be a total of at least 100 points.
One primary document must be submitted from section (A) which is 70 points and other documents from section (B) which make up the 30 points.

- An identification card issued by the Commonwealth, a State or Territory as evidence of the applicant’s entitlement to a financial benefit
- An identification card issued to a student at a tertiary education institution

- Membership card for a registered club

- All 3rd year student nurses must bring their Student ID card, transcripts and clinical summaries showing proof of their graduate year
- Non-residents must bring their passport
- Proof of Immigration/ Visa status for non-residents
- APHRA Nurses registration details
- Tax file number
- Bank account details (BSB and Account number)
- Superannuation details /img//misc/super_choice_form.pdf
- CV – resume with two nursing professional referees names and addresses
- A record of your hours worked as a nurse, Certificate of Service, a “Blue Book” or a Statutory Declaration

- A current First Aid Certificate
- NSW Police Clearance Check
- Immunisation History, i.e, NSW Health Record Card and please refer to the NSW Health policy for all new and existing staff "Occupational
