Heartbeat Nursing Agency will be provided with a variety of information, from a range of sources about you and your history. The information that you provide on the agency application for casual employment form for Heartbeat Nursing Agency will be used for the following purposes:

- To ensure compliance to government legislation - Police / Criminal record checks.
- To conduct reference checks on your qualifications and expertise.
- To match your skills and availability with the client specifications / booking.
- To ensure that you have the right to practice as a health professional.
- To ensure that you are fit to perform the duties of an agency nurse.
The information you supply will not be supplied to any third party, under any circumstances unless directed by government legislation or to obtain work on your behalf with the clients of Heartbeat Nursing Agency, other employment opportunities or for statutory reporting / requirements such as superannuation administration or banking requirements. The completion of the agency application form provides your consent to approach any named party / referee to verify the information that you have provided.
Heartbeat Nursing Agency will have procedures and policies in place on how information is handled, who is allowed access to your information, where the information will be stored, managed and retained.
If you wish to review the information that Heartbeat Nursing Agency has retained about you and your agency experience, then you will need to apply in writing to the Privacy Coordinator, Heartbeat Nursing Agency or obtain an information access application form from the office and complete all details. Incomplete or ambiguous applications may cause the request to be rejected. The application process does not guarantee automatic access to the information.
The request is reviewed and if accepted, then the information is provided to the requesting person, if not, then the request is refused. Any request to change information must be supported in writing with substantiated records / documentation to validate the request. This will be reviewed and once accepted, the database will be updated with the supplied information.
Heartbeat Nursing Agency has developed procedures, policies and forms for the management of the agency information and to ensure compliance to the Privacy legislation; and on an ongoing basis, the agency will review the implementation / operation of the agency in complying with the legislation.
